My personal story with alcohol

In skincare of course. ;) When I was fourteen, my skin started doing what most fourteen-year-old skin does. Breakouts. And like a lot of teenagers, I went looking for the strongest, most "serious" looking product I could find. I landed on a clarifying toner. The bottle promised to clear breakouts and purify the skin, and it had that sharp, cold, slightly stingy feeling when I swiped it on. To me, that sting felt like proof it was working.

It was not working. It was doing the opposite.

I only understood why years later, when I learned to read ingredient lists. I went back and looked at that toner, and there it was: alcohol, sitting near the very top of the list. Not buried at the bottom. Near the top, which (as you will see in a minute) means there was a LOT of it. My skin was oily and breaking out, so I had soaked it in something that stripped it, dried it, and pushed it to produce even more oil to compensate. The breakouts got worse, not better.

That toner is the reason I wanted to write this post. Because "alcohol" on a label is not one thing. And once you understand what it actually is, you stop being scared of the word and start reading it properly.

Why "alcohol" is one of the most confusing words on a label

Here is the core of the confusion. In chemistry, "alcohol" is not a single ingredient. It is a whole family of molecules that share one small feature: a specific oxygen-hydrogen group (an -OH group) hanging off their structure. That is it. That is the only thing they have in common.

So a tiny, volatile molecule that evaporates in seconds and a big, waxy molecule that feels like a moisturiser are BOTH technically alcohols. They behave completely differently on your skin. One can dry you out. The other softens you up. And they both show up on labels with the word "alcohol" in the name, which is exactly why people get scared of the wrong ones and miss the ones that matter.

To keep it simple, I am going to split them into three groups, because that is really all you need:

  • The drying alcohols (also called simple, low molecular weight, or "bad" alcohols). These are the ones the reputation is about.
  • The fatty alcohols (the big, waxy ones). These are emollients. They are good for most skin.
  • The in-between ones (aromatic alcohols like benzyl alcohol). Mostly there as preservatives or fragrance, usually in small amounts.

Before we go through each group, we need one quick lesson, because it changes how you read everything.

First, how an ingredient list actually works

This is the single most useful thing to know, and almost nobody teaches it.

Ingredients on a cosmetic label are not listed randomly, and they are not listed by how important they are. By law (this is how it works under FDA rules in the US, and the EU system is the same idea), ingredients have to be listed in descending order of concentration down to 1%. So the first ingredient is the one there is most of, the second is the next most, and so on (FDA Cosmetics Labeling Guide; 21 CFR 701.3).

There is one catch. Once you get below 1%, brands are allowed to list those ingredients in any order they like. So everything under 1% becomes a bit of a jumble at the bottom of the list.

Why does this matter so much for alcohol? Because where alcohol sits on the list tells you roughly how much is in the product. Alcohol as the second ingredient (like my old toner) means it is a huge part of the formula. Alcohol sitting down near the bottom, after a long list of other things, means there is barely any. Same word, completely different story. Hold on to this, because we will come back to it.

The drying alcohols: the ones the bad reputation is about

These are the small, light, fast-evaporating alcohols. They are the ones people mean when they say "alcohol is bad for your skin."

How to spot them on a label

Learn these names, because these are the ones to actually look for:

  • Alcohol (just the word on its own, which usually means ethanol)
  • Alcohol Denat. (denatured alcohol, very common)
  • SD Alcohol followed by numbers and letters, like SD Alcohol 40 or SD Alcohol 40-B
  • Ethanol or Ethyl Alcohol
  • Isopropyl Alcohol (also written Isopropanol)
  • Methanol (you should basically never see this in a real product, more on that later)

Quick note on "denatured." Alcohol Denat. and SD Alcohol are just ethanol with a tiny amount of a bitter or toxic additive mixed in so that nobody is tempted to drink the product (INCIDecoder). The codes after SD Alcohol (like 40-B) just tell you which additive was used. For your skin, treat Alcohol Denat. and SD Alcohol the same as plain ethanol.

What they are actually doing in there

Brands do not add these by accident. They are useful, which is why they are everywhere. The main jobs:

  • They are excellent solvents. A lot of skincare actives and fragrance ingredients will not dissolve in water. Ethanol dissolves them, so it lets a formulator put those ingredients into a product in the first place.
  • They make a product feel light and "fast." Ethanol evaporates quickly, so it gives that weightless, dry-touch finish and that fresh cooling feeling. This is why it is so common in toners, mists, gel sunscreens, and men's aftershave.
  • They are penetration enhancers. This is the important one. Ethanol can help other ingredients get deeper into the skin. It does this partly by dissolving and reorganising the lipids (the natural oils) that hold your skin barrier together. A major review of penetration enhancers describes exactly this: alcohols like ethanol disrupt the lipid packing in the outermost skin layer (Williams & Barry, 2004, DOI). Laboratory studies have shown that ethanol can extract significant amounts of those barrier lipids out of the stratum corneum (Sugibayashi et al., 1992, DOI; Vaddi et al., 2003, DOI).
  • They are mild degreasers and astringents. They cut oil and leave skin feeling tight and matte, which is exactly why they end up in "clarifying" products aimed at oily and acne-prone skin. Like my old toner.

Why they can be a problem, and for whom

Here is the catch with that penetration-enhancing, oil-dissolving talent. The thing that makes ethanol useful is the same thing that makes it risky: it is happy to dissolve YOUR skin's own protective lipids, not just the ones in the bottle.

Your skin barrier is basically a brick wall. The cells are the bricks, and a mortar of lipids (ceramides, cholesterol, fatty acids) holds them together and keeps water in. When a high concentration of a drying alcohol sits on your skin regularly, it can pull at that mortar. The short-term result is that tight, squeaky, slightly stingy feeling. The longer-term result, with repeated use, can be a weakened barrier: more water escaping, more dryness, more sensitivity, and ironically (for oily skin) more oil production as the skin tries to recover. You can see this clearly in a setting where alcohol is used a lot: alcohol-heavy hand products are well documented to cause dryness, cracking, and peeling with repeated exposure (Mishra & Vuppu, 2023, DOI). Now, this is not the same for everyone. This is the part most "alcohol is poison" posts get wrong. It depends on your skin and on how much alcohol is actually in the product:

  • Dry, sensitive, mature, eczema-prone or rosacea-prone skin, or a barrier that is already compromised: this is who should be most careful. Your barrier is already working hard, and a high dose of drying alcohol can tip it over. These are the skin types where a high-alcohol toner or essence is most likely to backfire.
  • Oily and acne-prone skin: this feels like the group that can "handle" it, because the matte, degreased finish feels great in the moment. But (speaking from fourteen-year-old experience) stripping oily skin can trigger more oil and more irritation. You can tolerate a small amount, but a high-alcohol product as a daily staple is usually not your friend.
  • Normal, resilient skin: generally fine with occasional or low-concentration exposure, especially in rinse-off or fast-evaporating products. A well-formulated product where alcohol is low on the list is rarely an issue.

Isopropyl alcohol is not the same as ethanol

People lump these two together, and they are related, but they are worth separating because you meet them in different places.

Ethanol (the alcohol in drinks, and the one in Alcohol Denat. and SD Alcohol) is the one you will see most in actual skincare formulas, as a solvent and that light-finish carrier I described above.

Isopropyl alcohol (isopropanol, the rubbing alcohol in your medicine cabinet) is a stronger degreaser and disinfectant. In cosmetics you tend to find it in things like astringent toners, some nail and hair products, and as a cleaning solvent rather than as a skincare carrier. On the skin it is at least as drying and defatting as ethanol, sometimes harsher, so the same cautions apply, just more so. If you see Isopropyl Alcohol high on an ingredient list of a leave-on product, that is a flag worth noticing.

One thing that catches people out: Isopropyl Alcohol is not the same as Isopropyl Myristate or Isopropyl Palmitate. Those last two have "isopropyl" in the name but they are emollient esters, basically lightweight skin-softening oils, not drying alcohols. The word "isopropyl" alone does not make something a drying alcohol. You have to read the whole name.

And one more, because it confuses everyone: Methanol is genuinely toxic and is not a legitimate skincare ingredient. It only ever turns up as an illegal contaminant, which happened with some fake or badly made hand sanitisers (Mishra & Vuppu, 2023, DOI). You should not see "methanol" on a real cosmetic label. If you ever do, put it down.

The fatty alcohols: same word, completely different molecule

Now for the plot twist. There is a whole other group of alcohols that does the exact opposite of drying your skin, and they are extremely common. These are the fatty alcohols.

The difference is in the size and shape of the molecule. The drying alcohols are tiny and light, so they flash off your skin and take oil with them. Fatty alcohols are big, waxy molecules made from natural fats and oils (like coconut or palm). They are heavy, they do not evaporate, and they feel creamy. Same -OH chemistry, totally different behaviour.

How to spot them on a label

These are your green-flag alcohols. Learn the names so you do not panic when you see the word "alcohol" next to them:

  • Cetyl Alcohol
  • Stearyl Alcohol
  • Cetearyl Alcohol (a blend of cetyl and stearyl, super common)
  • Cetostearyl Alcohol (another name for the same kind of blend)
  • Behenyl Alcohol
  • Myristyl Alcohol and Lauryl Alcohol

An easy rule of thumb: if the word in front of "alcohol" sounds like it comes from a fat or a wax (cetyl, stearyl, cetearyl, behenyl), it is a fatty alcohol and it is almost certainly there to be nice to your skin.

Why they are actually good for your skin

Fatty alcohols do three useful jobs, none of which involve drying you out:

  • Emollient: they soften and smooth the skin surface and help it feel comfortable.
  • Emulsifier and stabiliser: they keep the oil and water parts of a cream from separating, so your moisturiser stays a moisturiser and does not split.
  • Thickener: they give creams and lotions that rich, cushiony texture.

The independent Cosmetic Ingredient Review panel assessed cetearyl, cetyl and stearyl alcohol and concluded they are safe as used in cosmetics, functioning as emollients and stabilisers (CIR Final Report, fatty alcohols; Cosmetics Info, Cetyl Alcohol). The key line worth remembering: unlike ethanol, which is drying to the skin, fatty alcohols are not. They are basically the opposite ingredient wearing a similar name.

The only caveat: a small number of people with very sensitive skin can be irritated or mildly allergic to fatty alcohols (and to the closely related lanolin alcohol, sometimes listed as wool alcohols). If your skin is reactive and you are troubleshooting, it is worth knowing they exist. But for the vast majority of people, seeing cetearyl alcohol in a moisturiser is a good sign, not a bad one.

The in-between ones: benzyl alcohol and friends

There is a third little group that does not fit neatly into either box: the aromatic alcohols, mainly Benzyl Alcohol (and you will sometimes see Phenethyl Alcohol).

These are mostly there as gentle preservatives and as part of fragrance, almost always in small amounts (so usually low on the list). Benzyl alcohol is widely used and, despite occasionally being flagged, it is actually a rare cause of allergy. A large analysis of nearly 71,000 patch-tested patients found a positive reaction in only about 0.2%, and most of those were weak, leading the authors to conclude it is not a significant contact allergen (Geier et al., 2022, DOI). For most people it is nothing to worry about.

And a quick myth-buster while we are here: Phenoxyethanol is a preservative you will see constantly, and although the name looks alcohol-ish, it is not one of the drying alcohols. Same goes for ingredients that simply end in "-ol" (like glycerol, panthenol, tocopherol). The "-ol" ending does not make something a drying alcohol. You have to recognise the actual name.

Here are the most common look-alikes that trip people up. None of these are drying alcohols, so do not flinch when you see them:

Position on the list changes everything

Now let me bring it all back to the lesson from the start, because this is the skill that ties everything together.

When you see a drying alcohol (alcohol denat., ethanol, isopropyl alcohol) on a label, the first thing to check is not whether it is there. It is where it is.

Fatty alcohols are different: seeing cetearyl or cetyl alcohol fairly high up in a cream is completely normal and is part of what gives it a nice texture. No need to flinch.

When high alcohol content actually makes sense

I do not want to leave you thinking alcohol is always the villain, because there are formulas where a high dose is genuinely the right choice. This is the nuance the scare-posts skip.

  • Hand sanitiser. The clearest example. To actually kill germs, a sanitiser needs a high concentration of alcohol. Public health guidance recommends at least 60% alcohol for it to work (CDC hand hygiene). Here the high alcohol is the whole point. The dryness is a known trade-off you manage with hand cream, not a formulating mistake.
  • Some fluid and gel sunscreens. A lot of the lightweight, fast-drying, no-white-cast sunscreens people love are built on alcohol. It helps the filters spread evenly, dries quickly, and avoids that heavy greasy feel. For many people the cosmetic elegance is what makes them actually wear sunscreen every day, which matters more for your skin than the alcohol.   • Certain active serums and spot treatments. Some actives simply will not dissolve or penetrate well without ethanol as a solvent and carrier. Targeted acne spot treatments and astringents use it deliberately to degrease and to deliver the active. Used as a small spot treatment rather than all over the face, that can be a sensible trade.
  • Advanced delivery systems. There is a whole class of formulations called ethosomes that use a high concentration of ethanol (20 to 45%) on purpose, because the alcohol makes the delivery vesicles flexible enough to carry actives deeper into the skin, including for acne and other skin conditions (Paiva-Santos et al., 2021, DOI). In other words, sometimes the penetration-enhancing effect is exactly what you are paying for.

The point is not "high alcohol is always bad." The point is that high alcohol should match the job. In a sanitiser or a spot treatment, it makes sense. In a leave-on toner or essence you are pressing all over your face twice a day, especially with dry or sensitive skin, it usually does not.

What to look for

No brand names here this time, just the practical habits that will make you better at reading any label than most people:

  • Scan for the drying alcohol names first: Alcohol, Alcohol Denat., SD Alcohol, Ethanol, Isopropyl Alcohol. Those are the only ones worth hunting for.
  • Check the position, not just the presence. Near the top in a leave-on product is the real flag. Near the bottom is usually fine.
  • Match the alcohol to the product's job. High alcohol in a hand sanitiser or a targeted spot treatment makes sense. High alcohol in a daily, all-over leave-on toner is the one to question, particularly for dry or sensitive or barrier-compromised skin.
  • Do not panic over fatty alcohols. Cetyl, stearyl, cetearyl and behenyl alcohol are emollients. They are a good sign in a moisturiser.
  • Ignore the look-alikes. Phenoxyethanol, isopropyl myristate, glycerol, panthenol and tocopherol are not drying alcohols, even though the names look related.
  • If your skin feels tight, stingy or squeaky after a product, listen to it. That sting is not a sign it is working. It is usually a sign your barrier is being stripped. Fourteen-year-old me would have saved a lot of breakouts by knowing that.
  • If you do use a higher-alcohol product, support your barrier around it with a good moisturiser containing humectants and barrier lipids, so you are putting back what the alcohol takes out.

OK Emma great, now what's the actual takeaway?

Here it is, as simple as I can make it.

"Alcohol" on a label is not one ingredient, it is a family. The drying alcohols (alcohol denat., ethanol, isopropyl alcohol) can strip and weaken your barrier when they are present in high amounts, and they hit dry, sensitive and barrier-compromised skin the hardest. The fatty alcohols (cetyl, stearyl, cetearyl) are gentle emollients that most skin loves. Same word, opposite effect.

The skill is not memorising a blacklist. It is reading the label properly: find the drying alcohols, check how high up they sit, and ask whether that much alcohol makes sense for what the product is meant to do and for the skin you actually have. A drying alcohol near the bottom of the list, or in a quick spot treatment, is usually nothing. A drying alcohol as the second ingredient in a toner you swipe all over your face every day is the one that turned my breakouts into a bigger problem. So the next time a product stings and you think "good, it's working," I want you to remember a fourteen-year-old me and her clarifying toner, and turn the bottle around instead.
